I've edited the accent " ´ " into the apostrophe " ' " and "in thousands of pieces" into "in a thousand pieces".
But this line:
"it's snowing with tears on my heart" still sounds weird.
Could you explain that with different words?

I don't know how to put it in other words, because it is a figure of speech.
I think that the man wrote this was so sad, that his heart was frozen, and even his tears transformed into snowflakes...

I don't think he is dying, but I'm sure he feels he is reaching an end. It could mean he is feeling useless, or done, or gone or...many other things, because it's a beautiful but strange text.
