It's not good Dutch.. probably written by someone who's learning.. I can make sense of some things, but I have no clue what is meant by 'suders':

Dear 'suders'
has on mondays and 'urydag/ Fridays??' physical education and has to bring:
1. a pair of shorts
2. a t-shirt
3. a towel
4. clean underwear

Suders has to be "ouders"

Maybe it was written by hand so the "O" looked like an "S"?

Dan veronderstel ik dat urydag, vrijdag is?
Zal het gelijk proberen te vertalen.
