The statement is attached, you can have it filled in there (have Interflex fill it in). In addition we need proof of your job activities (what kind of work you were doing) in Poland until November 2006. (the question mark shouldnâ€™t be in the request) Please fill in the form and send it back before 16 May 2008. (Literally: would you please send the filled-in form backâ€¦) If we donâ€™t hear back from you we will start your request from 14 May 2007.
