Well...it's not wrong. Actually, it could be understood that way, however I would say that it's an imperative sentence addressed directly.
To me it means:
hi,
if the sentence translated to the Arabic must have a then pronoun thats ok, although I prefer that it is: "she smiles inspite of everything" , but also "she smiles despite everything" is ok.

la frase en si es un consejo: "sonria a pesar de todo" pero si es posible tambien me gustaria la traduccion de: "ella sonrie a pesar de todo", gracias
