ho bisogno de te sempre, senza di te non so cosa fare. Ma devo andare avanti perchè questo amore non mi porterà a niente. devo rendermene conto anche se sto male, ma piano piano passerà e starò meglio.

I will always need you, without you I don't know what to do.

English

Target language: English

I will always need you, without you I don't know what to do. But I have to go on because this love won't lead me anywhere. I have to understand even if I'm feeling bad, but slowly it'll go away and I'll be better.

I think "I need you always" is better.

Looks good to me, except for two issues:
bisogno = need
sto male = I'm not feeling well

"I'm bad" means a totally different thing

"ho bisogno de te sempre" -> "I always need you"

"se sto male" -> "if I'm ill" (or something alike)

I have to agree with goncin here, and request that you upgrade the English. In "meaning only" the English is good enough, but on second thought, you have a better grasp of English than you have written here, and, out of respect for your proven ability, I think it should be edited.
