I don't understand everything but I will set a poll in the hope that people's comments will make this text more meaningful!
I am particularly puzzled by 'scream and worm ream, and cult'.
If anyone can explain the text or offer a translation that explains the meaning, please do so!

kind a metaphorical poetry, I guess.
I'm not sure about how to express it in English,but I'll try to make it clearer.

"I’m life: suffer(/pain), faith, scream(/cry) and warm rhyme(?), and guilt(/guiltiness). I'm notating(/writing down) this short(/swift) moment before I become a silence."
(except two, all used as a noun)

not sure,but IO 'warm rhyme' may be refers to 'a short and pleasant poetry'.

Figen's suggestion is mostly a different way of expressing what you said, apart from
- worm ream -> warm rhyme
- cult -> guilt
