Kredi kartı üzerinde ki isim Babam'ın ismidir , Soyisimlerin uyuştu görülmektedir. Ek olarak gönderdiğim sürücü belgesinde Babamın adı belirtilmiştir, Eğer sorun yaşanması durumunda Babamın sürücü belgesi ya da kimlik copyasını gönderebilirim.

Yes, but there are some lines that don't sound very natural

"It is seen that surnames are in coherence with each other."
I think it should be something like:
"As you can see, (both) surnames match"

"In addition, my father's name is/appears (-) in the driving licence (-) I sent" or
"Moreover, it's my father's name in the driving licence I sent"

I think "Ek olarak gönderdiğim sürücü belgesinde" should be translated as "In the driving license which i enclosed/attached". What makes me think so is the lack of a comma after "ek olarak". Thanks.
