In english, the spanish verb "perseverar" is "to persist" or "to persevere". Then, the period "y pienso constantemente en ti" isn't correct.

For me that's close to the correct way...

That person is telling: ''Te amo y te extraño demasiado te necesito'' = ''I love you and I miss you so much I need you''

literal translation would be in my opinion: I hold on and think about you all the time because you are what I need.

I find it difficult to determine the right translation for this but the translation as it is now does not seem right to me

I persevere and I am constantly thinking about you because you are my necessity /because I need you.

there's missing: "...because..." and something more..
