Yes, potential should be included in the translation. F.ex. "...dies of smoking, maybe."

If you read the comments below the original text, you'll see that there is nothing dialectal in the sentence - just two mistakes/typos. I hope the original would be changed to:
"Vain joka toinen kuolee tupakkaan."
Please erase the remarks, too.

Then there is no need for "maybe" in the translation and it is fine like it is.

My suggestion:
Only every second person might be death/ might die by smoking cigarettes.
"Kuollee" ei ole murteellinen sanonta, vaan sanasta kuolla aktiivi, potentiaali, yks. 3. persoona.

"kuollee" is is kind of -ish form. in my mind the right translation should be "may die"
