This would be in the case that he might have upset her.

OR:

I am sorry that I have upset you.

This is the case that he has upset her and knows it.

But then the translation will pass without the "that" and/or "if" it just makes things a bit clearer to the reader.

The original text reflects the second alternative you have suggested. Besides, I learnt at the English courses the word "that" can be very easily let out and that's what I did here.
