I Googled the source and found that, among other things, "Vnitřní lékařství" is the title of a medical journal that also goes by the English title "Internal medicine".

Cisa, "physician" is not a medical specialty, but a generic term for any MD who is in clinical practice (meaning, treats patients). Internist is closer but still ambiguous.
