Well, there is difference in Russian.

You are always in my heart (when I sleep, when I eat and walk you are there as always). Refers more to present.

You are in my heart forever (you will be there forever and ever, for good and all, you came to stay). Refers to eternity.

I think it's simple, always vs. forever
