It's "(I) live with (my) heart", though pronouns are ommitted.

The meaning roughly is "I live by means of my heart".
"With" doesn't mean "together", it's the same to "I write with my hand or see with my eyes".

Ooops! You're right. I don't know why I translated it in the imperative. Perhaps it was the similarity to ancient Greek what confused me...

I think everybody lives with his/her heart, but here probably it should be translated as "I live listening to my heart"
