I don't think that 'Tomer and I haven't heard from you' is a word to word translation, but I don't know how to write in a different way. 'You've disappeared from Tomer and I' doesn't sound good. Maybe you could think of a way?

Sweetheart of mine is barely English, so I think we might settle for honey, or just sweetheart - both convey the meaning, sort of. Also type - 'from' not 'form'. Just for clarification, that bit of the sentence would literally be 'You've gone missing on Tomer and myself.'
