It's : "Hi, how are you doing? (What about you?/ You? )Are you going to send me a picture of you?".
I think he has already send her his picture (or that she has seen them in some way) and now he asks if she is going to send her too. "Do you want..." wouldn't be wrong in general (in both cases he asks for a picture) but we would miss the "exchange part" (it looks like it's her turn to send) that is implied.
