الحب الضائع إلى من عاش في أعماقي رغم أن حبه لي قد تلاشى إلى من أسكنته قلبي رغم الجرح الذي أصابني فيه
إلى من سيظل الأوحد في قلبي حتى وإن غاب عني إليه وإلى كل من يعانون مثلي أهدي هذه الهمسة
تتساءل أين وكيف ولمَ؟

to the one who lived deep inside of me although his love for me has vanished, whom I kept inside my heart in spite of the wound he caused, and who will remain the only one in my heart even when absent; to him and to whoever suffers like I do, I dedicate this whisper:

the problem is that the original lacks punctuation and sounds like it has a second part chopped out of it

This is better, but there are still some problems in English. For example, you can't say "to whom lived ..." - it has to be "to him who lived ..." This is true of all the cases where you have "to whom + active verb".
