Mother's birthplace (note that it looks as is it comes from a sort of formal -maybe goverment- form that wants information such as applicant's birthplace, his/her father's birthplace and mother's birthplace)

I think you should 'switch the words' here.
It looks more formal to put it like:

'Moderns födelseort'
